Frontenac Lennox and Addington Advisory Committee Out-Of-Home Respite for Children with Multiple Disabilities

Terms of Reference:

1.  Purpose:  The Out-Of-Home Respite Adviory Committee shall coordinate respite service access for:
-clients under 18 years of age and living at home
-children having more than one disability-related need resulting from a physical and/or developmental disability that requires support to participate in activities of daily living, school or play.
-children who are in significant need for service, the absence of which would lead to family breakdown, and /or harm to the child's self or others, and/or long term residential placement.

2.  Member agencies will include:
-Community Living Kingston and District
-Community Living North Frontenac
-Extend-A-Family Kingston
-Family Support Program, Family and Children's Services of Frontenac, Lennox and Addington
-Hotel Dieu Child Development Centre
-Ongwanada

3.  Function:
The Out-of-Home Respite Committee shall:
-Share information with the intent to determine agency response to individual needs.
-Determine eligibility if necessary
-Maintain a common list of eligible referrals.
-Determine lead agency for managing intake, assessment, and service delivery if required.
-Maintain Risk Assessment data for eligible clients, and monitor possible crisis situations.
-Monitor process with attention to standardized assessment and quitable responses.

4.  Referral Process:
-any agency on the committee can make a referral on a family's behalf.
