Registration Process

CHAP Support Providers promote active living for children and adults with autism, developmental disabilities, intellectual disabilities, and/or physical disabilities in or out of the family's home. Support Providers registered with the CHAP Support Provider Directory are connected with families looking for support providers.

Support Providers on the site are NOT
employees of respiteservices.com - CHAP Program and are considered self-employed, independent contract workers.

Some Key Responsibilities:

  • Provide safe, stimulating and recreational support both in or out of the family's home
  • Provide care and support according to the person's requirements
  • Provide respite/support services as requested by the parent/guardian
  • Network with community groups, facilitate inclusion and act as a positive role model, as required
  • Maintain communication with the Coordinator, respiteservices.com/Durham

Requirements:

  • You need to enjoy being with and supporting people
  • Understanding and respecting people with disabilities
  • Commitment to assisting people in building valued life experiences in or out of their home
  • Interest in supporting people to acquire and maintain skills and relationships
  • Experience providing care and support to persons with disabilities is an asset
  • First Aid and CPR is an asset
  • Capacity to keep and maintain records as self-employed contract worker and available for
    variable hours (days, evenings, weekends, overnights, relief, summer) to meet the needs of the person being supported
  • Minimum age of 18 years


How to Become a Community Helper for Active Participation (CHAP) Support Provider:

Step 1

Complete the Online Support Provider Registration and ensure that you include a classified ad in the space provided. 

  • This information is used by the families to select potential Support Providers. 
  • Upload a copy of your resume when creating your online profile

Step 2

You will be contacted by the Coordinator to arrange attendance at the mandatory half-day Information Session where you will sign the CHAP Program Referral Agreement, the CHAP Conduct and Expectations, the Police Information Check Policy, and the Oath of Confidentiality.

Please bring your Police Information Check for the Vulnerable Sector, a copy of your resume, and the contact information for your references (2 work-related & 1 personal but not a family member).  

If you cannot bring this information to the Information Session, you can submit it at a later date.
