The purpose of the CHAP Support Provider Program is to make the process of recruiting and contracting support providers easier for families. Once your registration is received, the Coordinator will contact you to confirm your request for support. The CHAP Support Provider Program will then be a resource to you for selecting your own Support Provider. The names of suitable people will be provided to you as they become available.
The CHAP Support Provider Program will not make referrals of support providers but will facilitate connecting families with various CHAP Support Providers based on the families' needs and the support provider's availability and skills. You independently engage the support provider. It is important to understand that a support provider is not an employee of the CHAP Program. It is up to you to select, interview, and engage your support provider(s) as Independent Contract Workers.
Please contact the Coordinator as soon as you have engaged a support provider, if your needs have changed, if you no longer wish to receive names but would like to remain on the directory, or if you would like to be removed from the directory. This will ensure that your information remains current and support provider profiles provided are those that match your current need.
If you have previously registered with respiteservices.com and have a received a username and password, please login to access your registration information.
If you are interested in accessing respite/support information and services in Durham, (including the CHAP Support Provider Program) and do not have a username and password, please complete the family online registration form.
